I give my full permission for him/her to attend the activity specified above that is sponsored by Grace Fellowship Church of Oak Forest. In checking this form, I now consent to administer any medical treatment that may be necessary in the opinion of a licensed physician or medical facility in case of any accident resulting in injury or illness that requires medical treatment during the activity. Every activity sponsored by Grace Fellowship Church is carefully planned and adequately supervised by mature adults. However, even with the best planning and precautions, unforeseen events occur. By checking this form, the parent or guardian agrees to assume and accept all risks and hazards in Grace Fellowship Church related to social activities. They also agree not to hold Grace Fellowship Church, its employees, or volunteer assistants liable for damages, losses, or injuries to the person or property named in this form.
